South Africa’s GLP-1 market shifted more rapidly in July 2026 than at any point since Ozempic was first registered here. Within a two-week period, two new approved semaglutide products entered the market, regulators issued a Class I recall of a major compounded product supplier, and the Gauteng High Court issued an interim order against the same supplier. Here is what it means for patients.
More approved options are coming: Sun Pharma’s SAHPRA-approved generic semaglutide and Novo Nordisk’s “Extensior” — distributed through Acino — both represent registered, quality-assured alternatives to branded Ozempic. These products go through the same SAHPRA registration process as Ozempic, meaning safety, efficacy and manufacturing standards have been assessed. Unregistered compounded products have not undergone this process.
Pricing clarity is still needed: The critical outstanding question is how much these products will cost versus branded Ozempic. If pricing is meaningfully lower, they could significantly improve access for patients who previously couldn’t afford the branded product. Official pricing for Sun Pharma’s generic and Extensior had not been published at time of writing.
Compounded GLP-1s are now in a precarious position: The recall of iDexis products and the High Court interim order effectively closes the largest compounded semaglutide supplier in South Africa. With approved lower-cost alternatives entering the market, the regulatory and legal argument for maintaining compounded GLP-1 supply — an affordability gap — is significantly weakened.
Patients on compounded products should see their doctor: If you have been using compounded semaglutide or tirzepatide from iDexis or any other supplier, consult your prescribing doctor before continuing. Recalled products should not be used. The registering of new alternatives means there are now more options to discuss.
