Novo Nordisk has announced it is partnering with Swiss pharmaceutical company Acino to launch “Extensior” in South Africa on 27 July 2026. Extensior is a lower-cost version of Ozempic (semaglutide), available in 0.25mg, 0.5mg and 1mg doses and using the same injection pen device as the branded product.
The move is a direct competitive response to Sun Pharma’s SAHPRA-approved generic semaglutide, announced in the same week. Novo Nordisk’s strategy — using a partner distributor rather than its own commercial channels — allows it to compete on price in a market segment it previously ceded to compounders and importers of unregistered products.
Extensior’s entry at 0.25mg, 0.5mg and 1mg doses covers the standard titration and maintenance doses used for type 2 diabetes management. These doses correspond to the diabetes indication rather than the higher 2.4mg weekly dose used for weight management (Wegovy).
For South African patients currently on branded Ozempic, the arrival of both Extensior and the Sun Pharma generic creates a more competitive supply landscape. Pricing details for either product had not been officially published at time of writing — the critical question for access is how significantly these products undercut the current Ozempic price.
