Discovery Vitality has broadened its HealthyWeight programme to include doctor-led clinical assessments and, where appropriate, prescriptions for GLP-1 anti-obesity medicines, as the insurer responds to a near-tripling in member claims for drugs such as Wegovy, Mounjaro and Ozempic. The move, announced on 1 October 2026, pairs medication access with dietitian consultations, behavioural coaching and rewards for healthy food purchases, in an effort to tackle one of the scheme’s most persistent findings: most members who start GLP-1 treatment stop within six months.
Claims for GLP-1 medicines have surged
Discovery Health Medical Scheme said claims for GLP-1 medicines climbed from 22,400 to 61,200 members in 2025 alone — a trajectory that has nearly tripled over the past three years, with the average member claiming for about 5.4 months of treatment. Vitality’s own data shows GLP-1 use among its members has tripled since 2022. The scheme frames the growth against a wider obesity burden: roughly one in two South African adults is overweight or obese, 70% of members classified as obese in 2010 were still obese 15 years later, and obesity carries a 52% higher mortality risk than a healthy weight.
A doctor-led, multi-pronged programme
Under the expanded HealthyWeight programme, members can access a doctor-led clinical assessment that may lead to a GLP-1 prescription, alongside dietitian support, behavioural interventions and incentive points for healthy food purchases. Enrolment starts at R295 a month. Discovery Vitality chief clinical officer Dr Mosima Mabunda said the evidence base supports combining medication with lifestyle change: “Research shows GLP-1 medicines are more likely to have sustained impact when accompanied by lifestyle changes,” she said, adding that “the question is no longer whether meaningful weight loss can be achieved, but how to help people sustain it.”
Discovery Vitality chief executive Dinesh Govender described obesity as “a chronic disease that often becomes harder to reverse over time,” and said the opportunity for the scheme is to intervene earlier rather than treat complications downstream.
Affordability is still the sticking point
Even as Discovery adds clinical infrastructure, cost remains the dominant barrier for most members paying for GLP-1s out of pocket. Mabunda acknowledged as much directly: “Cost is therefore an important barrier, particularly where treatment is paid for out of pocket.” Branded tirzepatide (Mounjaro, distributed in South Africa by Aspen Pharmacare under licence from Eli Lilly) generated an estimated R1.5 billion in local sales, while locally available generic semaglutide is expected to undercut branded pricing by at least 50% as more generics reach the market. Separately, financing specialist Obiflex has partnered with Novo Nordisk to offer interest-free instalment plans for GLP-1 treatment, with co-founder and chief executive Shaun Barns saying the company wants “to offer proper medical support, coaching and a way to pay for GLP-1s,” and the firm budgeting for a default rate of roughly 5%.
Early signs from the pilot
Discovery said a pilot phase of the HealthyWeight programme recorded a 46% increase in healthy food purchases among participants and a 25% reduction in unhealthy purchases, alongside average weight loss of 12–18% reported in clinical studies of GLP-1 medicines more broadly. The scheme is positioning the doctor-led pathway as a way to improve adherence and long-term outcomes rather than simply expanding access to the medicines themselves.
What this means for patients
- Discovery Health Medical Scheme members can now request a doctor-led assessment through Vitality’s HealthyWeight programme before starting a GLP-1 medicine, rather than accessing it through a standard prescription route alone.
- The programme bundles medication with dietitian support and behavioural coaching, reflecting evidence that lifestyle support improves how well weight loss is sustained after stopping or tapering medication.
- Out-of-pocket cost remains the main obstacle for non-Discovery members or those without scheme cover for GLP-1s; generic semaglutide and instalment financing options such as Obiflex’s Novo Nordisk–linked plan may offer lower-cost routes worth discussing with a treating doctor or pharmacist.
- Members considering enrolling should check their specific medical scheme option and benefit limits, as GLP-1 cover and the R295/month HealthyWeight programme fee vary by plan.
